Health Requirement Checklists
Many programs require a fieldwork experience (e.g. clinical, internship, practicum, etc.) as part of the course of study. Find your program-specific requirement checklist below. It lists everything you need to submit with resources conveniently hyperlinked.

Student Athletes are required to submit vaccination records (form listing requirements attached below).
- immunity to tetanus (vaccination within the last 10 years)
- immunity to pertussis (single dose of pertussis containing vaccine after age 18)
- immunity to measles (two live-vaccine doses or positive titer results)
- immunity to mumps (two live-vaccine doses or positive titer results)
- immunity to rubella (two live-vaccine doses or positive titer results)
- immunity to hepatitis B (three vaccine doses or positive titer results)
- immunity to chickenpox, known as varicella (two vaccine doses, positive titer results, or documented history of disease)
- evidence of a meningitis vaccine (Menveo or Menactra) after 16th birthday for all students between 16-21 years old
All Athletes must complete a one time Athletic Sickle Cell Trait NJCAA Form (PDF)
Athletes must also complete an annual Athletic Preparticipation Form (PDF)
Submitting Health Forms
Health requirement files must be submitted online using the appropriate Dynamic Form link below.
Name and date of birth required on all attached files-handwritten is acceptable. Student ID required. CORI must be notarized. Find a Notary Public at your bank or town office.
Once your files are reviewed an email notice will be sent listing remaining items due and/or subsequent action(s) required. Please monitor spam/junk.

Immunization Records
Vaccination records can be accessed using the Massachusetts Immunization Information System or the Connecticut Department of Public Health Portal.
If COVID-19 vaccination is on your checklist, you may request accommodation by submitting the Request for Reasonable Accommodations found on our COVID-19 Frequently Asked Questions page.
